Clinic Requirements:

1. It is very important that we have a head count prior to the clinic- Please return participation form to Coach Dennis Groll atcoachgroll@gmail.comor Ms. Gloria Louis at (337) 962-3009, via text or email @marialouis1@aol.com.

2. Association certification fee is $35.00per Olympiad Years (4years).Make all checks payable to Southern Association.

3. Individuals who are a part of an organization that will assume financial responsibility for your membership fees in addition to your certification fee, please make certain that a hard copied membership form accompanies the organization check. The total amount of the check for membership and certification fees is $65.00 per individual.

4. Individual must complete a USATF Membership Application online prior to the clinic. The cost is $ 30.00 a year. The application is online atmembership@usatf.orgor www.usatf.org. 5. Individual must complete their own Background Check ($16 for 2 years) online prior to the clinic or after. website:https://usatf.org/safesport/background-screening

6. Individual must complete a Safe SportTraining Course online prior to the clinic or after. Website:https://www.usatf.org/safesport/safesport-training. It is a free training course.

7. Additional information paper work and forms will be provided at the clinic.
